We would love to help. Any personal or medical information that you share with us is kept confidential. We follow guidelines offered by the Personal Health Information Protection Act.
To speak with an Occupational Therapist or schedule an assessment you have 1 of 4 options:
1. Complete our Online Registration Form:
Completing our online registration form can save time, especially if you know that you would like to go ahead and book an appointment. If you would like to speak with an Occupational Therapist before booking, you can indicate this in the form.
2. Book a phone appointment:
A parent can book a phone appointment directly with an OT via our system. You will need to create an account by entering your e-mail and a password. Then you will be able to select available options for an appointment that is convenient for you.
3. E-mail: firstname.lastname@example.org
Email Lizette, occupational therapist, directly to set up a phone appointment to discuss your concerns and available options for assessment and therapy for your child.
Concerns can also be discussed via e-mail, however, please note that e-mail communication is not considered secure as e-mail can be intercepted by a third party in the world wide web.
4. Call Directly: 647-236-4263
Please leave a voice mail with your name, phone number, and please indicate when you are available for a return phone call either today or tomorrow. An occupational therapist will call you back within 24 hours from Monday to Friday.
